Thank you for selecting CGFNS International for your credentials evaluation and verification needs. We noticed that your Credential Verification Service for New York State application has not been completed. Your application is incomplete due to the following missing item(s): 
    LICENSE AUTHORIZATION FORM
    TRANSCRIPT AUTHORIZATION FORM
    SIGNED NOTARIZED APPLICATION
In order to begin the verification process, CGFNS International requires that all the required forms and/or payment be received and accepted. If by March 1, 2016 we still have not received the necessary forms and/or payment, your application will be cancelled. You will not receive a refund. Once your application is cancelled, you will have to reapply for the Credential Verification Service for New York State and pay the full fee if you wish to have your credentials verified by CGFNS International.
