the increased attention to disaster preparedness in recent years has emphasized the important role that licensed professionals have in addressing critical needs when they arise. to help ensure that you can quickly and easily be identified as a New York State licensed professional as you carry out your professional activities day to day as well as in times of emergency, the NYS education department, in cooperation with the NYS department of motor vehicles(DMV), now offers all NYS licensed and registered professionals the opportunity to be issued, on a voluntary basis, a professional photo identification(ID) card.

the ID card will include your name, profession, license number, signature, DMV identification number, and an expiration date that will coincide with the expiration date of your professional license registration. We are pleased to offer these photo IDs based upon feedback we received from the State Boards for the Professions, professional associations, and others in response to requests for ideas on how we could strengthen the wervices we provide to NYS licensed professionals.

To request a photo ID, simply complete the Photo ID Application and return it with the required processing fee. If you have any quertions, please call the Photo ID Program Team at 1-800-567-7704. Thank you for your continued commitment to serving the public.
