공부도 해야 하는데 전 아직도 서류준비가 아직이네요~

ATT push 하려고 홈페이지 문의 하니 이런 메일이 도착했어요

궁금하기도 하고 막막해서 여쭤 봅니다~

Your application for licensure in Registered Professional Nursing has been forwarded to the Bureau of Comparative Education for a review of your education.  The Commission on Graduates of Foreign Nursing Schools (CGFNS) was unable to complete the verification of your education credentials.  In order for our office to proceed with the review of your education we require the following information:

·         Form 2F (form attached) - Certification of Foreign Nursing Education and an official transcript identifying the program completed (diploma program, associate degree program, or bachelor degree program) and the date the diploma or degree was awarded sent directly from your school of nursing (Hosan University) to this office.  An official transcript is issued by the school showing courses and grades and bears original signature and seals. 

While CGFNS received documentation of your educational credentials, the Office of the Professions requires clarification of the program completed (the Academic Record Form from the school indicated that you completed a diploma program, but the transcript and certificate of graduation indicated that you completed an Associate Degree in Nursing).  The Form 2F and the transcript must be submitted directly by the nursing school in an official school sealed envelope.  The Office of the Professions will not accept this form and/or the transcript if submitted by the applicant or any other party.

• Ph photocopy of your nursing diploma in the original language (Korean) and an acceptable English translation.

Request the school send Form 2F and the official transcript along with the diplomas via mail directly to New York State Education Department, Office of the Professions, Division of Professional Education, Bureau of Comparative Education, 89 Washington Avenue, Education Building, 2^nd Floor, West Wing, Albany, NY 12234-1000. 

When the Office of the Professions receives the additional information, this office will continue its review of the education portion of your application.  If you need clarification regarding what is being requested, you may contact this office at comped@nysed.gov, using my name, your profession, and your name in the subject line. Your e-mail should clearly state your question(s) and how I might contact you.

1. 1페이지 장은 제가 작성

    2페이지 장은 학교직원분이 작성 하나요?

   --->2번째 장에 직원이름 직책만 해달라 해야 하나요?(2페이지는 완벽하게 직원만 써야 하나요?)

2. 1페이지돠 2페이지 날짜가 같으면 되나요?

   --->내 서명 날짜가 하루만 더 빠르면되나요?(며칠 더빨라야 하나요?)

3. nurse form 2 양식 2장과 

    첨부 서류는 영문성적표 (직인) ,영문 졸업장(직인)

    ---> 이것만 보내면 되나요?

   서류에 학교 직인만 있음 되나요? 압인이 필요한가요

   압인은 어디어 찍어서 보내나요?

4. Photocopy of your nursing diploma in the original language (Korean) and an acceptable English translation.

   라고 되어 있는데요. 컬러로 되어 있음 무조건 흑백으로 복사해서 보내야 하나요?

  2페이지에 동그라미 안에  school seal 에 압인 찍고 옆에 인주도장은 학교 직인을 도장으로 찍으라는 말씀인가요?
